Health Requirements

Medical Requirements in the State of Illinois:


The medical requirements in the State of Illinois are as follows:

  • All students who are new to a school in Preschool through eighth Grade need:
  • Immunization records that are in full compliance with the State of Illinois.
  • A new medical exam not older than six months from the date of the first day of school.
  • A new dental exam not older than six months from the date of the first day of school.
  • All Kindergarten students must have an eye exam.
  • If an eye examination was given to a student in the State of Illinois, that must be submitted to the school. Out-of-state students must have an eye exam. If a student did not have an eye exam but is entering grades Kindergarten through eighth grade, an eye exam is required.
  • Students entering Preschool do not need an eye exam.
  • All students entering sixth grade must have a new medical exam, and dental exam, with update immunizations.
  • All students entering second grade must have a new dental exam.
  • Students who have allergies or need medication in school must have the Medical Authorization Form filled out by a doctor and parent. The medication is to be submitted to the school office with the child's name and grade on it. The medicine will be given to the classroom teacher.
